We are pleased to advise that registered Lax Kw’alaams Band members will receive a band gift, upon
satisfactory completion and submission of the required application form and supporting documentation.

The gift application is open and can be submitted immediately. Gift payments will start in December 2024. We appreciate your patience as we thoroughly process each application. For any questions, please

All fields must be complete using your full name as it appears on your status card, including middle
name, as this information is used to verify your identity. To apply on behalf of a minor (child under 18
years old), proof of custody may be required to prove that each child is fully in the care of the applicant.

To avoid processing delays, all fields must be answered completely by typing or printing each answer
clearly. Send completed applications and your bank payment information (direct deposit forms or void
cheque) to [email protected].

Eligibility
Eligible applicants must be:

• Lax Kw’alaams Band members, registered as such on August 31, 2024
• For minors under the age of 18 as of August 31, 2024 or at the time of applying, applications
  must be made by the parent or official guardian on their behalf
• Individuals over the age of 18 as of August 31, 2024, must make their own application, either
  individually or with a spouse
• Further documentation may be required if there is a shared custody arrangement in place.
  Parents may submit court documents outlining their custody arrangements
