Registered Lax Kw’alaams Band members are invited to submit applications for a one-time Band Member gift. Please read the Application 2023 Instructions for eligibility criteria.

The processing of applications will start in December 2023, on a first come first serve basis. To apply on behalf of a minor (child under 18 years old), proof of custody may be required. The amount of the gift is $750.

All applicable fields of the form must be complete using your full name as it appears on your status card, including middle name, as this information is used to verify your identity. To avoid processing delays, all fields must be answered completely by typing or printing each answer clearly. Send completed applications and your bank payment information (direct deposit forms or void cheque) to [email protected].

All applicants must complete and provide to Lax Kw’alaams Band, either:

• complete and submit the Online Gift Application available at this link or at the below QR code

• or, complete the attached fillable pdf “Lax Kw’alaams Band Gift Application 2023” using your device,
  • email this form to [email protected], or
  • print and drop off at the Lax Kw’alaams Band office in Lax Kw’alaams or the Lax Kw’alaams Business Development office in Prince Rupert

• or, complete a paper copy of the “Lax Kw’alaams Band Gift Application 2023,”
  • email this form as a PDF or JPG image (photo is ok) to [email protected], or
  • drop off at the Lax Kw’alaams Band office in Lax Kw’alaams or the Lax Kw’alaams Business Development office in Prince Rupert

Please be advised that the Band cannot accept applications by fax. Submissions by fax may not be legible when received at the Band office.

It is very important to have your complete contact information included on both forms, to ensure your gift is processed correctly, and allow staff to reach you quickly and efficiently if there is an issue with your application

Only use one method to submit your gift application. Duplicate submissions will result in delays of processing your gift. For those requiring more room for dependents, attach a second completed copy of the form together with your first application.

Direct Deposit

To expedite timely payments, the Lax Kw’alaams Band offers a direct deposit option. Applicants wishing to receive their gift as a Direct Deposit, must include an authorized void cheque or direct deposit form, issued by your bank, for the account you wish the deposit to be made. Most financial institutions offer a pre-populated void cheque/direct deposit form within their online banking sites. Alternatively, banks will provide these forms if you visit a branch and provide valid identification. Please ensure that your email address is shown on the void cheque/direct deposit form for cross-referencing purposes. All applicants requesting direct deposit must provide an email address.

A scanned version of your void cheque/direct deposit form must be submitted with the Online Gift Form, or emailed with your application form to [email protected].

For applications dropped off to the Lax Kw’alaams Band office or Lax Kw’alaams Business Development by paper copy, a paper copy of your direct deposit information will be required, along with the Electronic Funds Transfer Form (EFT) below.
