Gift Card Application 2021
Deadline Extended to July 30/21’
The Mayor and Council of the Lax Kw’alaams Band is pleased to announce a one-time gift card in the amount of $150 per band member living off reserve for COVID relief. If you have recently moved home and received a ‘Food Distribution’ in Lax Kw’alaams in the New Year and moved off reserve again, you will not be eligible for the $150 gift card.
Applications are available on-line at www.laxkwalaams.ca under the News tab or may be picked up at either the Band Office in the community or at Lax Kw’alaams Business Development office in Prince Rupert.
Eligible applicants must be Lax Kw’alaams Band Members registered as such on March 31, 2021. Further documentation may be required if there is a shared custody arrangement in place.
All applicants must be complete the attached ‘Gift Card Application 2021 – Off Reserve form’ in full. Use one application for all Registered Band Members living in household. Please print clearly to reduce delays in processing payments. Ensure your current and correct mailing address is included on the application as all gift cards will be mailed.
All applications must be either delivered to the Band Office during normal business hours, scanned and emailed to [email protected], or faxed to (250) 625-3246. Submit application by only one method only, as duplications of receipt creates delays in processing.
Final cut off has been extended to July 30, 2021.
Applications will be processed as they are received. It is crucial that the information is complete and clearly legible. Please do not call the office to enquire on the status of your application as this will just slow down the processing. Gift cards will not be replaced. We will not tolerate harassment of staff.
