Home and Community Care Nurse

The Lax Kw’alaams Band is seeking to find a Registered Nurse, part time to fill the Home & Community Care Program.

Job Responsibilities:

  • Provide comprehensive community health services (Home Nursing Care and Prevention) through home visits and other health promotion workshops;
  • Work with the Home & Community Care clientele
  • Participate as a team member with the FNHA
  • Keeps written records and statistics of program
  • Knowledge of competency guidelines, standards, scope and code of ethics
  • Skilled nursing services in accordance with the orders from the nurse practitioner and doctor under the plan of care. Staying within guidelines, in scope of practice.
  • Assist and support the clinic nurses with home visits as needed. Enabling immunocompromised clients to remain at home and receive knowledgeable safe care, of all ages.
  • Collaborating with family MD’s and NP’s for continuity of care.
  • Monitoring and assessing vital signs and blood sugar levels.
  • Assisting elders and community members medically as needed.
  • Preparation and or administration of medications including pre-pour medications and pre-loaded syringes. Monitor and care of catheters and ostomies.
  • Medication monitoring, support for clients with health conditions at home
  • Wound assessment and dressing changes in home, work in collaboration with the wound care team in Prince Rupert and clinic nurses
  • Follow Health and Safety principles in delivering care to clients in the community, working effectively and cooperatively with others.
  • Encourage and educate clients and families advocating on the behalf of the clients
  • Attend and participating in team meetings
  • Must be responsible and ethical, shows compassion and empathy.
  • Shows good time management and organization
  • Maintains client and staff confidentiality
  • Prepared to facilitate in community engagement workshops for Men/Women/Children and Families
  • Providing care plans for each client involved in the home and community care program
  • Other duties as required.

Qualifications

  • Must have a current RN Certification from BCNU
  • Experience in a First Nations Community
  • First Aid and CPR
  • Additional certification is preferred: Foot care/Palliative/Wound care
  • Minimum 5 years experience as an RN
  • Able to do in home assessments
  • Computer and office skills
  • Knowledge base in, Food skills for families and Community Health Education and Prevention
  • BSN Degree
  • 2 experience in Community Care
  • Practicing membership in the College of Registered Nurses of B.C.
  • Experience working with First Nations would be an asset
  • Provide acceptable references and criminal records check with vulnerable sector
  • Current BC Drivers License- Required

Compensation & Benefits

• Contract position
• Wages depending on Experience and Certifications
