Lax Kw’alaams Band is investigating the need for enhanced child-care services within the community. In order to fully understand the needs of families, we invite you to complete a community wide survey with your needs in mind.

We are pleased to provide you with access to the Child Care Needs Assessment – Parent Survey. The survey is now available to complete:

• On-line as a fillable form which can then be submitted electronically; or
• A paper version that can be picked-up and dropped off at the Administration Office; or
• A Lax Kw’alaams representative will endeavour to visit each home in the community between November 12th and December 9th , to ensure everyone has had the opportunity to complete the survey if they wish and to provide additional information on the Affordable Child Care Act.

Three gift certificates, $300, $200 & $100 will be drawn for, based on completed surveys. Individuals are able to submit their name one time only into the draw. Completing multiple survey forms will not result in multiple chances for the draw prizes.

All surveys must be received no later than December 9th in order to be counted and eligible for the draws.
